PATIENT RIGHTS AND RESPONSIBILITIES

Orthotic Prosthetic Associates, Inc. presents the following as your
rights and responsibilities as a patient. Please review them, and let us know if you
have any questions. You have the right
to:
- Receive considerate and respectful care.
- Receive prompt and appropriate care provided by qualified professionals.
- Receive reasonably full and complete information on procedures concerning
your care.
- Receive care without prejudice or discrimination because of race, color,
creed, religion, gender, marital status, national origin, sexual preference, handicap or
age.
- Confidentiality regarding your care with the ability to refuse release of
this information, except as required by law.
- Review upon request any patient records or information relating to your
care.
- Participate in decisions concerning your care with the ability to refuse
treatment, except when partial treatment could prove harmful.
- Voice any complaint or grievance without fear of threat or reprisal from
OPA or any member of our staff.
- Complete information on charges for services, the extent to which payment
for services may be expected from other sources, and the extent to which payment may be
required from you.
You have the responsibility
to:
- Provide honest, accurate and complete information about your medical
history that relates to the care that we provide.
- Be considerate of the rights of other patients and office personnel.
- Cooperate in maintaining a safe place to receive care.
- Arrive for your scheduled appointments or cancel in advance.
- Comply with your practitioner's recommendations and instructions,
including follow-up care.
- Tell your practitioner if your condition does not follow the expected
course.
- Tell us if you do not understand anything about your care.
- Ask questions about anything when you do not understand about your care.
- Ask sufficient questions until you do understand about your care.
- Tell us if you are not satisfied with your care.
- Notify our staff of any complaints or suggestions.
- Provide useful feedback about services and policies.
- Seek regular care during regular hours, and seek only emergency care
outside of regular hours.
- Make payment or arrange for payment of services provided.
